This community health center is a federally qualified health center (FQHC) dedicated to providing comprehensive primary care services to a diverse patient population in Northern California. The facility is committed to delivering high-quality, accessible healthcare to the community with a strong emphasis on continuity of care and patient-centered services.

Opportunity:
Advanced Practice – Physician Assistant – Family Practice, Family Nurse Practitioner

Job Details:

• Provide comprehensive family practice care in a Monday through Friday, 8am to 5pm schedule
• See a minimum of 18 patients per day with a mix of pediatric and adult populations
• Includes some Chronic Pain Management as part of patient care responsibilities
• Chart closure required within 72 hours
• Minimum of 3+ years of experience in a similar clinical setting required
• Active California license required; licensed in CA only
• Active DEA required before start date
• BLS certification required
• Board certification required (minimum certified)
• PAs must have a CA Controlled Substance Education Course Certificate
• NPs must have a Furnishing License with Schedules II
• Providers must be ORP (order, referring, and prescribing) enrolled in MediCal or willing to enroll during credentialing
• Candidate CVs must include month and date on all work experience with all employment gaps accounted for
• Required immunizations before start date include: 2022-2023 Flu Vaccine, TB Status/X-Ray Results (within 90 days of start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, and Tdap Immunization (within the last 10 years)
• Credentialing timeframe is approximately 45-90 days (six weeks standard for locums who submit application and documents timely)
• A 30-day notice is required for ending all assignments
• Nearest airport is ACV – Arcata

Compensation:
Competitive hourly rates with weekly pay and malpractice included.
