Opportunity:

Job Details:
- Per-diem opportunity providing face-to-face visit coverage and on-call coverage for hospice patients with serious, life-limiting illnesses
- Conduct face-to-face encounters required for hospice eligibility and recertification
- Shifts are flexible to accommodate APRN availability and family schedules for in-person visits
- Participate actively in weekly Interdisciplinary Team (IDT) meetings alongside physicians, nurses, social workers, chaplains, and other team members
- Provide clinical guidance and consultation to field staff
- Communicate effectively with patients, families, and caregivers regarding prognosis and plan of care
- Facilitate transitions across care settings including home, hospital, and skilled nursing facilities
- Coordinate with primary care and specialty providers to ensure continuity of care
- Complete timely and accurate documentation in compliance with CMS hospice regulations
- Ensure adherence to hospice Conditions of Participation and organizational policies
- Support quality metrics and documentation related to goals of care and advance care planning
- Provide on-call support including triage and symptom management guidance, with a minimum of 4 weeknights per month
- Participate in weekend and holiday coverage models as needed
- Must be a graduate of an accredited Nurse Practitioner program with board certification (FNP, AGNP, or Adult NP preferred)
- Active Connecticut APRN license with prescriptive authority required
- Minimum 2-3 years of clinical experience preferred
- Prior experience in hospice, palliative care, geriatrics, or home-based care strongly preferred
