This PACE (Program of All-inclusive Care for the Elderly) facility is part of a well-established community health organization dedicated to providing comprehensive care to elderly patients, helping them maintain independence and live safely within their communities. The facility operates as part of the San Diego PACE Program and is committed to delivering high-quality, patient-centered care through an interdisciplinary team approach.

Occupational Therapist

Job Details:

• Serve as a member of the Interdisciplinary Team (IDT) for the San Diego PACE Program, working Monday through Friday, 8am to 5pm
• Design and implement a comprehensive occupational therapy program to help PACE patients remain independent and live safely in the community
• Perform patient assessments to determine functional abilities and develop individualized plans of care aimed at decreasing disability and maximizing independence
• Supervise and ensure qualified support staff are following individualized patient care plans
• Guide patients and caregivers in the use of adaptive devices and therapeutic, creative, and self-care activities to improve function
• Conduct home, hospital, and other site visits, assessments, and interventions as directed by the IDT and/or Center Manager
• Evaluate the effectiveness of treatment plans and modify goals as necessary, performing reassessments as indicated
• Accurately record, sign, and date treatment notes, quarterly progress notes, evaluations, and discharge summaries in patient health records
• Ensure occupational therapy services are delivered in a therapeutic setting that promotes patient dignity and self-esteem
• Manage equipment and supply needs for the occupational therapy program
• Maintain compliance with all applicable regulations and standards including PACE, CMS, and DHCS requirements
• Participate in staff and caregiver training programs and attend required meetings and committees
• At least one year of experience as an OT working with geriatric populations is required
• Bilingual English/Spanish is preferred
• Must be a graduate of an OT curriculum accredited by the Committee on Allied Health Education and the Accreditation of the American Medical Association and the American OT Association
• Must hold an active Licensed Occupational Therapist license in California, a valid CA driver's license, reliable transportation, and proof of current vehicle insurance

Compensation:
Temporary employment is available based on an hourly rate determined by the employee's experience level on a 0-14 year scale. Please contact us for more information.
