Facility:
This outpatient clinic is a PACE (Program of All-inclusive Care for the Elderly) facility dedicated to providing comprehensive care to frail elderly populations. The facility emphasizes coordinated, community-based care and offers a collaborative work environment with a strong multidisciplinary team approach to patient care.
Opportunity:
Registered Nurse – PACE Admissions Outpatient Clinic
Job Details:
- Temporary 3-month assignment working Monday through Friday, 8:00am – 5:00pm
- Work under the direct supervision of the PACE Admissions RN Supervisor
- Responsible for development and timely submittal of the Level of Care (LOC) tool to the organizational shared drive
- Conduct initial In-Home Assessments (IHA) as part of the IHA Team, requiring travel for home site visits
- Assess home care needs of frail elderly populations using the nursing process and develop initial plans of care
- Complete and submit LOC determination forms to the Admissions RN Supervisor for review
- Review medical records as needed to complete LOC determinations, utilizing alternative sources such as hospital discharge information and medications when records are unavailable
- Maintain an up-to-date active list of all submitted LOCs, including submission dates, decisions, and follow-up information
- Follow up on all deferred Prospective Enrollees and resubmit required information to DHCS to request approval
- Coordinate implementation of homecare services including Durable Medical Equipment for program participants when needed
- Assist with transitioning of care and monitoring of hospitalizations and SNF patients to ensure appropriate transitions
- Communicate and interact with patient caregivers and/or family members as needed
- Participate in weekly enrollment team meetings as well as IDT and other meetings as needed
- At least 1 year of RN experience in home or community health with the frail or elderly population required
- Current California Registered Nurse license required
- Current BLS for Healthcare Providers CPR or CPR/AED for the Professional Rescuer certification (American Heart Association) required
Compensation:
Temporary employment is available based on an hourly rate determined by the the employee's experience level on a 0-14 year scale. Please contact us for more information.
