This opportunity is with a highly integrated healthcare system in Connecticut, featuring a broad network that includes seven acute care hospitals, one of the largest academic and surgical hospitals in the northeast, thriving community teaching hospitals, and one of the largest multi-specialty medical groups in New England with more than 1,000 providers. Opportunity:
Physician – Hospice

Job Details:

  • Participate in the decision-making process related to admission to hospice care, both at home and in the inpatient setting
  • Serve as a member of the Interdisciplinary Team in developing individualized patient care plans
  • Certify and re-certify patients eligible for hospice care under the Medicare Hospice Benefit
  • Consult with attending physicians regarding pain and symptom management
  • Coordinate efforts with the attending physician to ensure continuity of care
  • Develop and monitor the medical component of the hospice program
  • Participate in the development, coordination, and implementation of hospice services and policies
  • Participate in the development of staff and volunteer training programs
  • Conduct home visits, both in-person and virtual, as a consulting physician when appropriate
  • Act as a hospice liaison and advocate with national, state, and local physicians, healthcare professionals, partners, and the community
  • Perform related administrative functions as required and/or assigned
  • Schedule: 48 on-call weeks per year, Monday 7am through Friday 7pm, including mandatory in-person attendance at one Interdisciplinary Team Meeting (IDT) per week; four weekends per year covering Friday 7pm through Monday 7am

Compensation:
Competitive hourly rates with weekly pay and malpractice included.
